A structured approach
What the Navigation Process May Look Like
The process begins with the client’s priorities and proceeds only within the agreed scope of service and the permissions provided.
1
Identify the Immediate Priorities
We clarify what needs attention now, what can wait, and what remains uncertain.
2
Build an Organized Action Plan
We create a practical list of tasks, responsible parties, deadlines, and needed information.
3
Obtain Written Permissions
Before contacting another person or organization, Crescendo confirms the client’s authorization and communication preferences.
4
Support Coordination and Follow-Up
We help prepare communications, track responses, and identify unresolved next steps.
5
Review and Adjust
We review what has been completed, what remains open, and whether the support plan should change.
Client-directed coordination
Collaboration Requires Clear Permission
Crescendo does not contact providers, schools, agencies, employers, family members, or other third parties without the client’s permission, except when disclosure is required or permitted by applicable law.
When coordination is requested, the client will be asked to complete the appropriate authorization or release. The client may limit what information can be shared, with whom it can be shared, and how long the authorization remains in effect.
Navigation remains client-directed. Clients may revoke an authorization or change their communication preferences as permitted by the service agreement and applicable law.
Clear service boundaries
What Client Navigation Is Not
Client navigation is a nonclinical coordination and organizational support service. It is not:
Medical, psychiatric, psychological, legal, financial, or educational advice
Psychotherapy, counseling, diagnosis, or treatment
Medication management or interpretation of medical instructions
Crisis response, emergency dispatch, or continuous on-call supervision
Case management provided under a government or insurance program unless separately authorized and contracted
Guardianship, power of attorney, legal representation, or authority to make decisions for the client
A guarantee that a referral, appointment, benefit, accommodation, or service will be approved or available
Privacy-conscious support
Information Is Shared on a Need-to-Know Basis
Navigation may involve sensitive personal, educational, or health-related information. Crescendo uses the minimum information reasonably necessary to complete the authorized task.
Clients should avoid sending sensitive information through ordinary text messages, social-media messages, or other channels that have not been approved for confidential communication.
Specific privacy practices, communication methods, record handling, and authorization requirements are addressed in Crescendo’s service agreement, privacy notice, and release forms.
Emergency limitation
Navigation Services Are Not Emergency Services
Crescendo does not provide emergency monitoring or immediate crisis response through navigation services. Do not use email, voicemail, text messaging, or the client portal to report an emergency. Call 911 for an immediate emergency. In the United States, call or text 988 for the Suicide & Crisis Lifeline when appropriate.
Create a clearer path forward
Let’s Organize the Next Steps
You do not have to keep every appointment, form, phone call, and follow-up task in your head. We can begin by identifying what feels most urgent and building a practical system for moving forward.
Crescendo Behavioral Health’s client navigation services are educational, organizational, and supportive in nature. They do not constitute medical care, psychotherapy, diagnosis, treatment, legal advice, financial advice, emergency services, guardianship, or authority to make decisions for the client. Coordination with third parties is subject to the client’s written authorization and applicable law.
